1964 Ears Custom IEMs and Remolding Service
Believe it or not, you can buy someone else's custom IEMs and have them rebuilt into your own custom mold. Almost as surprisingly, you can take a standard universal fit IEM--like the Shure SE535 or UE Triple-Fi 10--and have their innards installed into your own custom ear pieces. 1964 Ears is one of the companies that provides this service. Cost is about $200 (not including the ear impressions you must have from an audiologist, which typically run about $60).

A full line of brand new custom IEMs are also available from 1964 Ears ranging in price from $325 to $650. I listened to their lowest cost product and thought it sounded pretty darn good for a $325 in-ear. Add the fact that it's custom fit, and you've got a real bargain.

Good company

To expand on what Tyll said - 1964 Ears is able to reshell any IEM, turning it into a custom molded unit just for you. The only limitation is that it must be a balanced armature design rather than a dynamic driver. 

Got a universal IEM with a sound you love, but you aren't thrilled about the comfort? This would be something to consider. 

I have also been impressed with their own designs. They have a "house sound" that is really smooth on top without being too dark or rolled-off. Great for fans of LCD-2, HD650, etc.
